Appointments

Patients are seen in the office by appointment only. For your initial visit, please arrive 15 minutes early to complete necessary paperwork and check-in process with the front desk. You can access our new patient forms below and we encourage you to complete them prior to your visit.

Call Reminder

You will typically receive a call two days prior to your appointment to remind you of your upcoming visit. Please make sure to update your preferred phone number with the staff for confirmation.

A Note On Wait Times

We strive to run an efficient practice and we recognize that you are busy. We will make every effort to notify you in the unlikely event of a significant delay resulting from a problem or emergency of some kind. We apologize in advance for any inconvenience you may incur and we ask for your understanding, as we would provide you with the same undivided attention in the case of such an unfortunate event.

5 Things to Remember

Arrive 15 minutes early to complete all necessary paperwork.
Bring your insurance card.
Co-payment is due at the time of service, unless otherwise stated. We accept cash, checks and the following credit cards: Visa, MasterCard, American Express, and Discover.
Bring a credit card. You will be required to submit a credit card in order to pay for all outstanding balances on future visits.
Bring any records from your previous physician, including but not limited to: recent physical, latest blood work, vaccine history, list of active specialist(s), diagnostic testing - EKG, Stress Test, Echo, Colonscopy, CT Scans, MRI’s.

Appointment Cancellations

Fairfield Medical Group has a 24-hour cancellation policy. Please provide at least 24-hours notice for appointment cancellations or you may be billed. This fee will not be reimbursed by your insurance.

$50.00 fee for missed physical examinations, annual wellness visits or pre-operative appointments.
$30.00 fee for missed follow-up and problem visit appointments.
