*Due to HIPAA Compliance laws, we are not capable of communicating through direct email. You can contact our office with any inquiries or requests to make an appointments during normal business hours, which are listed below for your convenience. Our friendly staff is always ready and eager to assist you. You may also request an appointment through your MyChart patient portal through Yale New Haven Health Systems. 

*If you have not set up a MyChart portal, please call our office at 203-255-8827 to request an activation code. Once that code is received, there are simple, easy to follow instructions to create your account. Should you lose your password or get locked out of your account, please send an email to mychartsupport@ynhh.org.

*Please call our office between 9:00 am and 5:00 pm for all routine matters. If you have true medical emergency after regular office hours, you can reach the on-call physician. You can call our office phone number to reach the on-call provider after hours. You can also leave a message for less urgent matters. In the unlikely event that our phone system malfunctions or you do not receive a timely response, please go to the nearest emergency room.
